Home-Based Mental Health Evaluation (HOME) Program

The Home-Based Mental Health Evaluation (HOME) program is a suicide prevention intervention for Veterans aimed at increasing treatment engagement and decreasing suicide risk following psychiatric hospitalization, a period of heightened suicide risk. Veterans in the HOME program receive a phone call the first day post-discharge, a home visit during the first week and weekly phone calls thereafter until they are engaged in outpatient mental health care. Each contact typically includes: thorough suicide risk assessment, review and modification of Veteran’s safety plan and discharge plan/upcoming appointments and problem-solving around barriers to care. Data suggests that Veterans who participate in the HOME program are more likely to engage in care, engage in care more quickly and attend more outpatient appointments.
